Abstract

Background:The management of tooth fractures depends on the severity, pulpal involvement, and the condition of the surrounding periodontal tissues. In fractures involving dentin and extending to the pulp, a comprehensive treatment approach is required to preserve the tooth within the oral cavity. Objective: This case report aims to describe the comprehensive management of tooth fractures through a combination of root canal treatment and restorative procedures, as well as to review the clinical outcomes and therapeutic considerations underlying the selection of treatment. Case Report: A 21-year-old female presented to RSGM Soelastri with a complaint of a fractured upper anterior tooth that had occurred approximately six years earlier due to trauma from hitting a pole. At present, the tooth was asymptomatic. Based on objective examination, tooth 22 showed a crown fracture involving the pulp with negative on probing, positive on percussion, negative on palpation, and negative on vitality. Case Management: The diagnosis in this case was Ellis Class IV fracture of tooth 22, accompanied by a periapical lesion. The treatment plan included patient education (KIE), root canal treatment, post and core placement, indirect restoration using a jacket crown, and follow-up evaluation to assess treatment success. Conclusion: Root canal treatment followed by fiber post placement and a jacket crown showed favourable outcomes, indicated by the absence of symptoms and improvement of the periapical lesion. This combination therapy is effective in eliminating infection, preserving the tooth, and restoring both function and esthetics, thereby providing a good long-term prognosis.
