Background: Enamel hypoplasia is a permanent defect of dental enamel that can affect both primary and permanent teeth, with multifactorial etiology including metabolic stress, genetic factors, environmental influences, and trauma. Clinically, this condition is characterized by pits, grooves, and thinning or loss of enamel, with a prevalence of around 2% in Indonesia, making it relatively rare. Case Report: An 8-year-old male patient presented with an aesthetic concern in the anterior tooth without pain. Intraoral examination revealed a cavitated lesion with a vertical groove extending to the gingiva on the labial surface of tooth 11. The diagnosis was enamel hypoplasia, with differential diagnoses including fluorosis, enamel caries, and erosion. The treatment plan included parental education and direct restoration using composite resin. Case Management: A2 composite resin is used for cases of enamel hypoplasia on maxillary anterior teeth and post-filling control of A2 composite resin so that it still looks aesthetic. Conclusion: Enamel hypoplasia is often identified incidentally, highlighting the importance of thorough clinical and radiographic evaluation to support accurate diagnosis, ensure long-term treatment success, and prevent further complications.
