Background: Multiple diastema refers to spaces formed between several teeth, caused by dental anomalies such as tooth size or number, wide dental arch, tooth loss, high frenulum attachment, and tooth migration due to physiological or pathological factors. Dental crowding occurs when there is an imbalance between the available space and the space required for the teeth, resulting in overlapping teeth. This condition is more commonly found in the mandibular teeth. Objective: To demonstrate that a simple active plate in a removable orthodontic appliance can be used to correct maxillary multiple diastema and crowding of the mandibular anterior teeth. Case Report: A 24-year-old female presented with complaints of crowding in the lower teeth and gaps between the right and left maxillary anterior teeth, which caused food impaction and led to a lack of self-confidence. The patient had no significant general health issues. Case Management: The treatment was performed using a removable orthodontic appliance, which involved retracting the maxillary anterior teeth and correcting mandibular tooth malposition through a combination of interproximal enamel reduction. Conclusion: The use of a removable orthodontic appliance with a simple active plate in this case was proven to effectively close gaps and correct individual tooth malpositions, with treatment progress evaluated using the PAR index showing a 67% improvement.
